Steps to Receive Transaction Approval

 

Option #1 - Online and/or Mobile Form

 

1) Obtain appropriate Forms from your Investment Provider (financial company that manages your retirement account), fill it out and sign.

 

2)Use this online form to request TPA Authorization for Loans, Distributions (including Hardship and Unforeseen Emergency), Contract Exchanges, Plan to Plan transfer, and Plan Rollovers In/out. Fill it out.

3) Upload a copy or picture of your vendor paperwork, then submit.  You will receive a confirmation email when complete.

4) 403b Partners (the plan administrator) will forward approval certificate and all relevant documents to the Investment Provider for final processing. A second confirmation email will be sent once all information is approved and submitted to your investment provider.  Please contact your financial company directly regarding the status of your request.

Option #2 - Traditional Printable Forms

1) Print your appropriate Transaction Form or Salary Reduction Form.  Use this form to request TPA Authorization for Loans, Distributions (including Hardship and Unforeseen Emergency), Contract Exchanges, Plan to Plan transfer, and Plan Rollovers In/out. Fill it out and sign.

2) Obtain appropriate Forms from your Investment Provider (financial company that manages your retirement account), fill it out and sign.

3) Submit the form and all relevant documents to 403b Partners (the 403b/457 Plan Administrator) for approval.

4) 403b Partners (the plan administrator) will forward approval certificate and all relevant documents to the Investment Provider. Please contact your financial company directly regarding the status of your request.
